Evaluation Setup
This feature will facilitate the setup of forms for the evaluation process.
✑ From the OSIS sidebar, Click on the Lecture Evaluation link under Assessment Management and then, on the Evaluation Setup link.
It comes with two(2) buttons to the top right of the page. Find Form and Add Form.
To create a new evaluation form, click on the Add Form button. The Add form modal has the following sections:
- Add Evaluation Form: Enter the name of the form (a name you can easily identify your form with).
- Add Section: The way various categories your question will be grouped under.
- Add Questions: The questions for each section on your form.
- Preview Form: A window to allow for the validation of the form setup before saving.
Click on Create Form to proceed to the next step.
✑ Click on Continue to proceed to the next step.
✑ Click on the Add Questions button to add questions to each section.
✑ Click on the Add from Instruction to add an instruction to the form.
For each form created, there is an option to Manage(edit) the details, view the form, and edit or delete it.
✑ Click on Manage to manipulate the entries of the setup
✑ To edit a form, click on the edit link under the modify button.
This will bring up a form to edit the form name and change the form instruction(s).
✑ To delete a form, click on the delete link under the modify button.
An alert is brought up to confirm the delete action. Click on Delete Form to delete a form.
